Pet Food Distribution Registration Requirements
Everyone is required to reregister annually and to provide the following information:
· Rabies
· Dog license
· Description of animals
· Current driver’s license or state id with current address
· Proof of income for the entire household – documentation must indicate actual income/award amounts. Access cards alone are not acceptable.
Clients will need to re-register annually. Records will be compared to past registration. Increase in the number of animals is not permitted.
